What BMI do you need for Wegovy or Mounjaro?

If you are exploring prescription weight loss in the UK, BMI is usually the first eligibility question. Wegovy (semaglutide) and Mounjaro (tirzepatide) are the most discussed weekly injections, and they share similar BMI thresholds under obesity licensing.

This guide explains the numbers, how they apply to both medicines, when lower cut-offs apply, and what happens after you pass the BMI screen.

BMI criteria for Wegovy and Mounjaro

For adults starting obesity treatment in the UK, prescribing generally follows NICE-aligned thresholds:

  • BMI 30 or higher (obesity), or
  • BMI 27 or higher (overweight) with at least one weight-related health condition

Weight-related conditions commonly include:

  • Prediabetes or type 2 diabetes
  • High blood pressure (hypertension)
  • High cholesterol (dyslipidaemia)
  • Obstructive sleep apnoea
  • Established cardiovascular disease

These cut-offs apply to both once-weekly semaglutide (Wegovy) and once-weekly tirzepatide (Mounjaro) for weight management when licensed criteria are met. BMI alone never guarantees a prescription: contraindications, pregnancy, certain cancers, pancreatitis history, and other factors matter too.

Is eligibility different for Wegovy vs Mounjaro?

On BMI, usually no. The difference between them is mechanism, dosing, side effects, and individual response, not a separate BMI ladder. Compare options in Mounjaro vs Wegovy and read our doctor's guide to Wegovy or what is tirzepatide (Mounjaro) for treatment-specific detail.

Switching between medicines, restarting after a break, or continuing at maintenance dose all need a fresh clinical review; see restarting treatment after a break and maintenance support.

BMI for Wegovy Pill, Nevolat, Mysimba, and orlistat

Not everyone starts on weekly injections. Other PrivateDoc options include:

  • Wegovy Pill (oral semaglutide, where licensed): broadly similar obesity BMI principles to injectable semaglutide
  • Nevolat (daily liraglutide): typically BMI 30+ or 27+ with weight-related comorbidity when prescribed for weight management
  • Mysimba and orlistat: separate licensing rules; often BMI 30+ or 27+ with a weight-related comorbidity

Why your BMI matters (and its limits)

BMI estimates body fat from height and weight. UK regulators and NICE use it as a practical screening tool for obesity medicines. It does not measure muscle mass or fat distribution, so athletes and very muscular people may have a high BMI without high body fat.

That is why clinical assessment always goes beyond a calculator result.

Lower thresholds for some ethnic backgrounds

NICE notes that people from some ethnic groups may face equivalent metabolic risk at a lower BMI. Clinicians may apply adjusted cut-offs (often around 2.5 BMI points lower) for backgrounds including:

  • South Asian
  • Chinese
  • Middle Eastern
  • Black African
  • African-Caribbean

PrivateDoc clinicians apply this context during consultation so assessment stays fair and personalised.
