Orforglipron activates GLP-1 receptors involved in appetite regulation, digestion and blood glucose control.

GLP-1 medicines can:

The effect on appetite develops differently from person to person. Some people notice changes relatively early in treatment, while for others the effect develops gradually as the dose is increased.

Foundayo should be used alongside a reduced-calorie diet and increased physical activity as part of a longer-term approach to weight management.

Clinical trials have shown that orforglipron can produce meaningful weight loss when used alongside diet and physical activity.

In the Phase 3 ATTAIN-1 trial, more than 3,100 adults with obesity or overweight took part across nine countries. Participants did not have diabetes.

After 72 weeks, weight loss varied according to the dose received.

*The 17.2 mg tablet corresponds to the highest orforglipron dose used in the clinical development programme.

Among people who remained on treatment and reached the highest dose, average weight loss was around 12.4% after 72 weeks.

Clinical trials also reported improvements in measures including waist circumference, blood pressure and some blood lipid measurements.

Results vary from person to person. Clinical trial averages cannot predict exactly how much weight an individual will lose.

Foundayo is licensed for weight management in adults with obesity, or adults who are overweight and have at least one weight-related health condition, alongside a reduced-calorie diet and increased physical activity.

A UK-registered prescriber must assess your individual suitability before treatment can be prescribed.

Your assessment may consider:

• Your BMI
• Your current weight and weight-management goals
• Weight-related health conditions
• Previous weight-loss treatment
• Your medical history
• Current medicines
• Potential contraindications
• Your ability to follow the treatment safely

Typical clinical criteria for weight-management medicines may involve a BMI of 30 or higher, or a BMI of 27 or higher alongside a weight-related health condition, although your prescriber will determine whether you meet the relevant licensed and clinical criteria.

Foundayo treatment starts at a low dose and is increased gradually to help your body adjust and minimise gastrointestinal side effects.

The dose escalation schedule is:

1. 0.8 mg once daily
2. 2.5 mg once daily
3. 5.5 mg once daily
4. 9 mg once daily
5. 14.5 mg once daily
6. 17.2 mg once daily maximum

A minimum of 30 days is spent at each dose before increasing.

Not everyone needs to reach the maximum dose. Your prescriber will consider how well you respond to treatment and how well you tolerate each strength.

The aim is to identify the lowest effective dose that provides an appropriate response while remaining tolerable.

Never increase your dose more quickly than instructed by your prescriber.

Like other GLP-1 medicines, Foundayo can cause side effects, particularly those affecting the digestive system.

Side effects are more likely when treatment is first started or after a dose increase. Gradually increasing the dose can help reduce their impact.

Common side effects include:

• Nausea
• Constipation
• Diarrhoea
• Vomiting
• Stomach or abdominal pain
• Bloating
• Indigestion
• Heartburn or reflux
• Belching
• Headache
• Tiredness
• Flatulence
• Hair loss

These effects are generally mild to moderate and often become easier to tolerate as your body adjusts.

Do not take Foundayo if you:

• Have a personal history of medullary thyroid carcinoma (MTC)
• Have a family history of MTC
• Have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
• Have had a serious allergic reaction to orforglipron or any of its ingredients

The prescribing information contains a boxed warning relating to thyroid C-cell tumours. Thyroid C-cell tumours have been observed in animal studies involving GLP-1 receptor agonists. It is not known whether this risk applies to humans.

Tell your healthcare professional if you develop a lump or swelling in your neck, persistent hoarseness, difficulty swallowing or difficulty breathing.

You should also tell your prescriber if you have:

• A history of pancreatitis
• Severe gastrointestinal problems
• Delayed stomach emptying or gastroparesis
• Kidney problems
• Gallbladder problems
• Diabetic retinopathy
• Significant liver disease
• A planned surgical procedure
• Any previous problems with anaesthesia

Tell your prescriber about all medicines, supplements and treatments you currently use.

Foundayo can interact with some medicines, while its effect on stomach emptying can also affect the absorption of some medicines taken by mouth.

Particular care may be required with:

Insulin and sulfonylureas

Taking Foundayo with insulin or sulfonylurea medicines can increase the risk of hypoglycaemia. Your diabetes treatment may need to be adjusted.

Simvastatin

Foundayo can increase exposure to simvastatin. The prescribing information recommends limiting simvastatin to 20 mg once daily when used with Foundayo.

Medicines affecting CYP3A4

Some medicines can increase or reduce the amount of orforglipron in your bloodstream.

Strong CYP3A4 inhibitors can increase orforglipron exposure. If a strong CYP3A4 inhibitor is used, the maximum Foundayo dose is limited to 9 mg once daily.

Strong CYP3A4 inducers can reduce orforglipron exposure and may make treatment less effective.

Your prescriber should check your medicines before treatment begins.

Other oral medicines

Because Foundayo delays gastric emptying, it can affect the absorption of some oral medicines.

Always provide a complete list of your current medicines during your clinical assessment.

Follow the storage instructions supplied with your medicine and on the dispensing label.

Keep Foundayo:

• In its original packaging
• Protected from direct light
• At the recommended room temperature
• With the container securely closed
• Out of the reach of children

Do not use the medicine after the expiry date shown on the packaging.

Both Foundayo and the Wegovy Pill are daily oral GLP-1 treatments, but there are important differences.

Both medicines have demonstrated meaningful weight loss in clinical trials.

Because the medicines have not been compared directly in the same clinical trial, their trial results should not be treated as a direct comparison of effectiveness.

For many people, the practical difference will be the administration routine: Foundayo does not require fasting, while the Wegovy Pill does.

Mounjaro contains tirzepatide, which acts on both GIP and GLP-1 receptors. Foundayo contains orforglipron, which acts on GLP-1 receptors.

Mounjaro is administered as a once-weekly injection, whereas Foundayo is a once-daily tablet.

These medicines have different mechanisms, dosing schedules and clinical evidence bases. They should not be considered interchangeable.
